PRO work registration for Commercials

I have music in a commercial and need to complete the BMI registration form (http://www.bmi.com/pdfs/commercial_jingle.pdf).
Does anyone have experience with this?
Especially looking for information on how to obtain the ISCI number.
Also in the form it asks for:
PERFORMANCE INFORMATION FOR AIRINGS OF THE COMMERCIAL IN LOCAL MARKETS. COMPETITRACK DATA WILL BE USED FOR PERFORMANCES IN MAJOR MARKETS.
Do I need to ask the ad agency for these? (I prefer like bug them as little as possible).
Any info would be highly appreciated.
